Self Reporting COVID-19
Self-Reporting for Students and Staff
Students and staff will be encouraged to self-report when they have been exposed to COVID-19, have symptoms of COVID-19, or have received a positive diagnosis of COVID-19. When students test positive, they will submit a notification through the district website. The student will be quarantine in accordance with the time identified by the CDC Quarantine Calculator with administrator approval. Upon return, students are encouraged to wear facial covering for 5 calendar days. Campuses will notify the district and custodial services upon confirmation of a positive student case, at which time, additional campus and classroom sanitation will take place.
Self-reports will be made using the following forms:
