Affidavit of Residency and Guardianship Support

Affidavits of Residency will now be completed by visiting the student’s campus of enrollment during normal school hours. 

Please ensure that both the parent/guardian who resides with the Duncanville ISD resident and the resident are present. The resident must provide proof of residency dated within 30 days prior to enrollment and a valid state/government issued ID with an address that matches the proof of residency. 

All Affidavits of Residency must be completed annually.

Guardianship

The birth parent designated on the birth certificate and the guardian should be present.

Guardianship forms can be completed by visiting the student’s campus of enrollment during normal school hours or at the Duncanville ISD Education Plaza, located at 710 South Cedar Ridge Drive, Duncanville, TX, 75137, during the following hours:

Monday, Tuesday, and Wednesdays, 9:00 AM - 12:00 PM

All Guardianship Forms will be uploaded into Skyward and must be completed annually.
