Affidavit of Residency

Affidavits of Residency can be completed by visiting the Duncanville ISD Plaza, located at 710 South Cedar Ridge Drive, Duncanville, TX 75127, to have the form notarized during the following times:

• Monday – Wednesday, 9:00 AM – 12:00 PM
• All Affidavits will be uploaded into Skyward and must be completed annually.
• Please ensure that the parent/guardian who lives with the Duncanville ISD resident and the resident are both present.
• The resident will need to provide proof of residency dated within 30 days prior to enrollment.
