Affidavit of Residency

Please complete the Affidavit of Residency for processing. Schedule an appointment with the campus to have the form notarized. All Affidavits will be uploaded into Skyward and must be completed annually.

If you are not able to access the form, please use the studentservices@duncanvilleisd.org email address or call the office of Student Services at (972) 708-2000 and provide the following information:

Name(s) of student(s) who live with the Duncanville ISD resident.
Name(s) of parent/guardian(s) who live with the Duncanville ISD resident.
Proof of residency of the Duncanville ISD resident.
