In compliance with Texas Senate Bill 12, Duncanville ISD must obtain written consent for your child to receive school-based health and emotional wellness services from qualified district staff. These professionals may include school nurses, counselors, social workers, athletic trainers, and other certified health and mental health providers.
By granting consent, your child will have access to timely and equitable services that promote health, safety, wellness, and readiness to learn. Without consent, staff will be limited in the support and services they can provide during the school day. A separate consent form must be completed for every Duncanville ISD student in your household. To fill out the consent form, please visit Skyward Family Access.
